Un tueur en série s’évade de prison et active un réseau de disciples prêts à tuer pour lui.Ryan Hardy, ancien agent du FBI qui avait arrêté Joe Carroll plusieurs années auparavant, est rappelé pour participer à la traque. Il découvre rapidement que le criminel a profité de son incarcération pour recruter des admirateurs et les transformer en exécutants dévoués. Face à cette organisation disséminée dans la population, Hardy tente de comprendre le nouveau plan de Carroll avant que ses adeptes ne frappent à nouveau.
